Are you attending Thursday's Export Control/ITAR OR the US Government Agencies and Contractors session?
All conference attendees planning to attend the export control/ITAR session must complete a visit request form verifying U.S. citizenship or U.S. permanent resident status. A PDF copy of the visit request form is posted on this webpage along with an example completed form. Multiple attendees may be certified on one form. The clearance and social security number sections of the form are not required and should not be completed. Once the form is filled out, the company facility security office or a corporate officer (president, vice president, etc. when no facility security officer is present) must sign verifying U.S. citizenship or U.S. permanent resident status. U.S. permanent residents must also provide a copy of their permanent resident card. The completed and signed form shall be faxed to (505) 846-7877. Completed forms must be received by COB 11 June. At the conference, each attendee must present a valid government issued picture identification card to gain access to the session. Valid U.S. passports may also be presented at the conference to gain entrance to the session and in this case, no visit request form is required. The visit request form verification method is preferred.
We have added a US Government Agencies and Contractors session to the MTD agenda in addition to the standard ITAR/Export Control closed session. Attendees for the "US Government Agencies and Contractors" session must modify the VISIT REQEST form, see example and add the words US GOVERNMENT or GOVERNMENT CONTRACTOR in the FROM block at the top of the VISIT REQUEST form. Therefore the person signing is certifying citizenship and government/contractor status. Those qualified to attend the US Government Agencies and Contractors session may also attend the ITAR/Export Control Session.
In summary, there are two choices for attending the June 18, ITAR/Export Control sessions:
1.Fill out the version for the ITAR/Export Control if they are US Citizens/US Permanent Residents but NOT US Government/Contractors
2.Fill out the version for ITAR/Export Control/US Government/US Contractor if they are both US Citizens/US Permanent Residents and US Government/Contractors
